Service User Feedback Survey

This survey can be given to service users by organisations funded through Community Solutions to gather feedback about their experience of the activity/service delivered, to support them to complete their evaluation and monitoring forms.
Funded projects can issue the survey as a word document to service users or provide them with the JotForm link. VANL will summarise responses to the JotForm link and share with each organisation.

Care Opinion is the UK's independent non-profit feedback platform for users of health and social care services. On Care Opinion, people can anonymously share their honest experience of a health care or social care service they have received within the last 3 years. Service user feedback/stories are directed to the people responsible for delivering the service, who respond in 7 days to the feedback. Where the feedback is critical, this also supports learning and improvements to the service.
All feedback/stories are reviewed by the Care Opinion team before they are shared to ensure anonymity and that the feedback/story reaches the right service/organisation.

Community Solutions are piloting the use of Care Opinion for the Community and Voluntary Sector whereby funded projects invite their service users to provide feedback on their experience of using the service to support ongoing learning and improvements.
