Description: The project focuses on supporting patients aged 65 and over who do not have many or any regular visitors, particularly those with long-term stays or facing challenging health conditions. The project encourages social interaction, alleviates feelings of loneliness, and fosters a sense of community within the hospital. Befriending support will be offered to the patients post discharge to help to support them to reconnect with their communities and help to avoid repeat admissions to the hospital.

Befriending Support for Men aged 65+

Description: Befriending service for people aged 65+ who are facing the effects of the impact of isolation and loneliness. These supports include one-to-one befriending, group befriending, telephone befriending, telephone wellbeing checks, regular events, exercise and wellbeing classes among other things.
