Fund Purpose
- The focus of this project would be on improving processes and networks to make social prescribing more efficient and capable of effectively linking people with the appropriate services to live independent lives. To achieve this a development officer will be recruited to work across North Lanarkshire’s third sector and health and social care landscape to implement the necessary changes
- The current social prescribing landscape in North Lanarkshire lacks uniform language and direction in terms of how it communicates and collaborates, and this manifests in difficulty knowing where to access the right services and support for patients and latterly how to facilitate access to that service or support
- At this stage the need lies in strengthening existing links and creating new ones to expand and enhance the social prescribing network across North Lanarkshire. This will involve making sure that pathways between third sector providers and practitioners are clearly defined and utilised effectively
